This handbook is an easy-to-use reference guide for district clerks and others who coordinate MUST enrollment for their school district. The handbook includes important information concerning:

Forms – The most current forms are required and can be on the MUST website, as well as on the district’s renewal thumb drive.


When submitting your monthly payment to MUST, please include the Billing Reconciliation sheet. It is important to verify that everything on the monthly premium detail and any retroactive adjustments are correct. If not, please contact our office at 1-800-845-7283 to make any necessary corrections. Any changes need to be done on the Universal Form and received by MUST within 60 days of the event.

If there is any question as to the available benefits or costs, please contact your agent, MUST representative or the MUST office. MUST offers medical, dental, vision, as well as life and LTD coverage.

The Summary Plan Description and other materials specific to group plans supersede this general information with regard to individual participants’ eligibility and benefits. You may wish to keep a printed copy of this document available as a convenient reference.
